Clinical applications of Cardiovascular MRI and Percutaneous Coronary Intervention

Details

My two areas of research interest are:
1) The clinical applications of CMR imaging, in particular for patients with ischaemic heart disease (both chronic stable angina and acute coronary syndromes). This includes the development and validation of comprehensive CMR protocols to assess left ventricular function, myocardial perfusion, tissue oedema and viability and coronary artery anatomy. I have applied these imaging protocols in both stable and unstable patients, and have demonstrated the safety and utility of CMR in NSTEMI and STEMI.
Current major projects include:
a) Assessment of the diagnostic accuracy of CMR vs. SPECT, in a large prospective patient cohort, with x-ray angiography as the reference standard, for the non-invasive detection of IHD. This includes a health economic evaluation of CMR and assessment of prognosis (The BHF funded CE-MARC study).
b) CMR assessment of myocardial tissue oedema, and the effect of potent systemic inflammatory agents at reducing myocardial damage and limiting ventricular dysfunction in patients with NSTEMI (The MRC funded ILA-HEART study).
c) Evaluation of the value of CMR imaging to predict the potential for improvement in LV function after percutaneous coronary intervention to occluded coronary arteries (Heart Research UK).
d) Development of vascular imaging protocols for the assessment of aortic distensibility, 4D flow imaging, plaque burden, pulse wave velocity and brachial artery flow mediated vasodilatation.
 
2) Percutaneous Coronary Intervention, in particluar the use of drug eluting stents in stable and unstable coronary artery disease. My PCI research interfaces with my CMR research in that CMR is now used as a end point measurement tool in many of our interventional clinical trials. For example, two of our NIHR-EME funded collaborations (The CVLPRIT trial and the MVO study). I also have a developing interest in using CMR to assess the outcome from structural heart disease intervention e.g. Transcatheter aortic valve implantation.
